From August 2026 public hospitals began charging foreigners who are not residents, with emergencies still free and at least one province declining to apply the policy. A legal resident is unaffected — which makes residence status, not money, the thing that decides access.

Pathoff outcome rubric (the Pathoff method, section 10-4), residency band "A route with conditions you can meet": Temporary residence as a rentista or pensionado is granted for up to three years and renewable, on recurring income from abroad of at least five times the minimum wage, leading to permanent residence. Mid-band: the threshold is modest and the path onward is short, but the funds must enter through banks authorised by the central bank, which in a country with currency controls is a real condition rather than a formality.

Migration rules were amended by decree in 2025 and the changes are still being implemented, so terms recorded elsewhere may already be out of date.

Pathoff healthcare rubric (the Pathoff method, section 13-2), band "Covered, with a private top-up in practice": Public hospitals treat legal residents without charge and private prepaid plans are affordable, which is what most people who can afford them hold; serious treatment is done in the country. At the foot of the band because quality varies sharply between provinces and the rules just changed.

Public hospitals treat legal residents without charge. Set against that: from August 2026 non-residents are billed for hospital care, provinces are applying that differently, and the living cost on this page is blank because the World Bank no longer publishes the data to fill it.
